A summary of our services:

Skilled Nursing — Specialized Help at Home: HomePointe HealthCare believes that through education, training and disease management programs, we can help patients, especially children, live the best life possible — at home.

"HomePointe has enabled us to maintain stability in our family and our home."

Skilled Private Duty Nursing: For complicated health issues or illness, RN or LPN private duty care is a valuable alternative to further hospitalization or a nursing home. HomePointe HealthCare provides private duty nursing from two-hours to 24-hour continuous care. Specialized care includes pediatric nurses who are experienced with ventilator-dependent children.

This service, for complicated health issues and illnesses, begins with a customized care plan that addresses the family's unique situation and encompasses physical, spiritual and emotional needs.

Nurses assigned to your loved one will collaborate with his or her physicians and other health care providers and focus on assessment, management, support, education and coordination of care.

While our nurses care for all patients of all ages, our specialty is children who are medically fragile and includes managing:

  • Premature and high-risk infants
  • Airway problems
  • Chronic lung disease
  • Diabetes
  • Failure to thrive
  • Post-surgery needs
  • Cardiac conditions
  • Wound care
  • Neuromuscular disease
  • Feeding problems
  • Tracheostomy
  • Gastrostomy/Jejunostomy tubes
  • Developmental delays
  • Seizure disorders
  • Neurological disorders
  • Other pediatric medical conditions

Rehabilitation Therapies: HomePointe HealthCare's specialized physical, occupational and speech therapists work to empower patients to resume or continue their daily activities and manage their own rehabilitation process. Therapists provide services to children at high risk for developmental delays due to marked prematurity or delays in gross motor skills, including children who face physical challenges, long-term disabilities or injuries.

"A supportive hand means a more fulfilled life for me and my family."

Skilled Intermittent Nursing: RN and LPN visits of two hours or less are available to provide care and training to manage recovery following surgery, cardiac events and stroke. Skilled nursing is also available to provide IV medication, wound care and guidance on nutrition, medications and general health care.

Home Health Aide: Skilled HomePointe HealthCare Aides provide assistance with hygiene, meal preparation, medications and Activities of Daily Living (ADLs).

Working under the supervision of a registered nurse, our home health aides provide basic procedures such as taking and recording vital signs, infection control, personal hygiene and range of motion exercises. Additionally, we train aides in basic nutrition and how to recognize emergencies. On a day-to-day basis, you will find our aides maintain detailed records of services and the client's condition, enjoy working with people, are reliable and maintain a clean, safe environment.
